Signed receipt for FMCSA Safey Regulations Pocketbook

Written authorization for driver to obtain his/her drug/alcohol testing history from previous employer(s)

Previous Pre-Employment Employee Alcohol and Drug Test Statement

Signed certificate of receipt for drug/alcohol company policy

A signed statement showing total on-duty time for the past 7 days (to verify compliance with hours-of-service rules

Authorization for PSP Report

TRANSIT GROUP TO COMPLETE:

Negative DOT pre-employment drug test result

Driving record from state where driver was licensed for last 3 years

Accident and drug/alcohol testing history from previous employer(s) from last 3 years, or document of good-faith effort to obtain that information

Documentation showing medical examiner was listed on the National Registry of Certified Medical Examiners at the time of the exam.
